Stem cell tourism is a small but growing part of the thriving global medical tourism marketplace. Much stem cell research remains at the experimental stage, with clinical trials still uncommon. However, there are over 700 clinics estimated to be operating in mostly developing countries – from Costa Rica and Argentina to China, India and Russia – that have lured many patients, mostly from industrialized countries, driven by desperation and hope, which in turn continue to fuel the growth of such tourism.While (...) much research has focused on such dimensions as the promotions that allow such businesses to make their services known, media coverage, some patient research, and regulatory conditions for developing country clinics, little attention has been paid to the non‐affected members of the general population, the future potential users of such services. This empirical study based on five focus group discussions with a diverse group of healthy adults in a Canadian city, explored participant views of patients who use stem cell tourism services, the likelihood they would avail themselves of such services if they were to suffer similar illnesses, and the conditions under which they might do so, and the impact that admonitions and advice from international expert bodies might have on their decisions. Our findings suggest that these healthy adults are sympathetic to the drivers of hope and desperation, and, despite cautions about research limitations, may seek such treatments themselves under similar conditions. Xenotransplantation, or the use of animal cells, tissues and organs for humans, has been promoted as an important solution to the worldwide shortage of organs. While scientific studies continue to be done to address problems of rejection and the possibility of animal-to-human virus transfer, socio-ethical and legal questions have also been raised around informed consent, life-long monitoring, animal welfare and animal rights, and appropriate regulatory practices. Many calls have also been made to consult publics before policy decisions are made. This (...) paper describes the Canadian public consultation process on xenotransplantation carried out by the Canadian Public Health Association in an arm’s length process from Health Canada, the ministry overseeing government health policy and regulation. Focusing on six citizen fora conducted around the country patterned after the citizen jury deliberative approach, the paper describes the citizen panelists’ recommendations to hold off on proceeding with clinical trials and the rationales behind this recommendation. The consultation process is discussed in the context of constructive technology assessment, a framework which argues for broader input into earlier stages of technology innovation, particularly at the technology design stage. Biotechnology and nanotechnology are both strategic technologies, and the former provides several lessons that could contribute to more successful embedding and integration processes for the latter. This article identifies some of the key questions emerging from the biotechnology experience and summarizes several lessons learned in the context of constructive technology assessment. A new generation of climate fiction called Cli-fi has emerged in the last decade, marking the strong consensus that has emerged over climate change. Science fiction’s concept of cognitive estrangement that combines a rational imperative to understand while focusing on something different from our everyday world provides one linkage between climate fiction and science fiction. Five novels representing this genre that has substantial connections with science fiction are analyzed, focusing on themes common across these books: their framing of the climate (...) change problem, their representations of science and scientists, their portrayals of economic and environmental challenges, and their scenarios for addressing the climate challenge. The analysis is framed through Taylor’s ideas of the social imaginary and the sociology of expectations, which proposes that expectations are promissory, deterministic, and performative. The novels illustrate in varying ways the problems attending the science-society relationship, the economic imperatives that have driven the characters’ choices, and the contradictory impulses that define our connections with nature. Such representations provide a picture of the challenges that need to be understood, but scenarios that offer possibilities for change are not as fully developed. This suggests that these books may represent a given moment in the longer trajectory of climate fiction while offering the initial building blocks to reconsider our ways of living so that new expectations and imaginaries can be debated and reconceived. Studies of multiple paternity in mammals and other animal species generally report proportion of multiple paternity among litters, mean litter sizes, and mean number of sires per litter. It is shown how these variables can be used to produce an estimate of the probability of reproductive success for a male that has mated with a female. This estimate of male success is more informative about the mating system that alternative measures, like the proportion of litters with multiple paternity or the (...) mean number of sires per litter. The probability of success for a mated male can be measured both theoretically and empirically, and gives an estimate of the intensity of sperm competition and of a male's “confidence of paternity” upon mating. The probability of success for mated males for ten “exemplar” species of mammals is estimated and they are compared for insights into the functioning of their mating systems. (shrink)

We ask when, for a pair of structures $\mathcal {A}_1,\mathcal {A}_2$, there is a uniform effective procedure that, given copies of the two structures, unlabeled, always produces a copy of $\mathcal {A}_1$. We give some conditions guaranteeing that there is such a procedure. The conditions might suggest that for the pair of orderings $\mathcal {A}_1$ of type $\omega _1^{CK}$ and $\mathcal {A}_2$ of Harrison type, there should not be any such procedure, but, in fact, there is one. We construct an (...) example for which there is no such procedure. The construction involves forcing. On the way to constructing our example, we prove a general result on modifying Cohen generics. Parentage analyses via microsatellite markers have revealed multiple paternity within the broods of polytocous species of mammals, reptiles, amphibians, fishes and invertebrates. The widespread phenomenon of multiple paternity may have attending relationships with such evolutionary processes as sexual selection and kin selection. However, just how much multiple paternity should a species exhibit? We developed Bayesian null models of how multiple paternity relates to brood sizes. For each of 114 species with published data on brood sizes and numbers of sires, we (...) compared our null model estimates to published frequencies of multiple paternity. The majority of species fell close to our null model, especially among fish and invertebrate species. Some species, however, had low probabilities of multiple paternity, far from the predictions of the null model, likely due to sexual selection and environmental constraints. We suggest a major division among species’ mating systems between those with close to random mating and high levels of multiple paternity, and those with constraints that produce low levels of multiple paternity. With a growing number of genetic tests becoming available to the health and consumer markets, genetic health care providers in Canada are faced with the challenge of developing robust decision rules or guidelines to allocate a finite number of public resources. The objective of this study was to gain Canadian genetic health providers' perspectives on factors and criteria that influence and shape resource allocation decisions for publically funded predictive genetic testing in Canada. The authors conducted semi-structured interviews with 16 senior (...) lab directors and clinicians at publically funded Canadian predictive genetic testing facilities. Participants were drawn from British Columbia, Alberta, Manitoba, Ontario, Quebec and Nova Scotia. Given the community sampled was identified as being relatively small and challenging to access, purposive sampling coupled with snowball sampling methodologies were utilized. Surveyed lab directors and clinicians indicated that predictive genetic tests were funded provincially by one of two predominant funding models, but they themselves played a significant role in how these funds were allocated for specific tests and services. They also rated and identified several factors that influenced allocation decisions and patients' decisions regarding testing. Lastly, participants provided recommendations regarding changes to existing allocation models and showed support for a national evaluation process for predictive testing. Our findings suggest that largely local and relatively ad hoc decision making processes are being made in relation to resource allocations for predictive genetic tests and that a more coordinated and, potentially, national approach to allocation decisions in this context may be appropriate. Both unipolar and bipolar depression have been linked with impairments in executive functioning. In particular, mood symptom severity is associated with differences in common EF, a latent measure of general EF abilities. The relationship between mood disorders and EF is particularly salient in adolescence and young adulthood when the ongoing development of EF intersects with a higher risk of mood disorder onset. However, it remains unclear if common EF impairments have associations with specific symptom dimensions of mood pathology such as (...) blunted positive affect, mood instability, or physiological arousal, or if differences in common EF more broadly relate to what is shared across various symptom domains, such as general negative affect or distress. To address this question, bifactor models can be applied to simultaneously examine the shared and unique contributions of particular mood symptom dimensions. However, no studies to our knowledge have examined bifactor models of mood symptoms in relation to measures of common EF. This study examined associations between common EF and general vs. specific symptom dimensions using structural equation modeling in adolescents and young adults with varying severity of mood symptoms. A General Depression factor capturing shared variance across symptoms statistically predicted lower Common EF. Additionally, a factor specific to physiological arousal was associated with lower Common EF. Anhedonia-specific and Mania-specific factors were not significantly related to Common EF. Altogether, these results indicate that deficits in common EF are driven by, or reflect, general features of mood pathology that are shared across symptom dimensions but are also specifically associated with physiological arousal. BackgroundPalliative sedation and analgesia are employed in patients with refractory and intractable symptoms at the end of life to reduce their suffering by lowering their level of consciousness. The doctrine of double effect, a philosophical principle that justifies doing a “good action” with a potentially “bad effect,” is frequently employed to provide an ethical justification for this practice. Main textWe argue that palliative sedation and analgesia do not fulfill the conditions required to apply the doctrine of double effect, and therefore (...) its use in this domain is inappropriate. Furthermore, we argue that the frequent application of the doctrine of double effect to palliative sedation and analgesia reflects physicians’ discomfort with the complex moral, intentional, and causal aspects of end-of-life care. ConclusionsWe are concerned that this misapplication of the doctrine of double effect can consequently impair physicians’ ethical reasoning and relationships with patients at the end of life. It is now known that the "Theology" is in fact not a work (...) of Aristotle's but rather a paraphrase of parts of Plotinus's Enneads. Certainly, the misattribution of this work to Aristotle facilitated the spread of Neoplatonism as an aspect of Islamic peripateticism, as represented by such authors as al-Kindi, al-Farabi, and Ibn Sina (Avicenna). Some efforts have been made to assess its influence on particular philosophers, most notably Ibn Sina whose notes on the "Theology" have come down to us and who may have doubted its authenticity. 1 Until recently the attention paid to the "Theology" itself has tended more towards philological than philosophical analysis, focusing in particular on possible sources of the text. 2 Yet the differences between the original writings of Plotinus and its Arabic paraphrase are of considerable philosophic interest. In recent years research into the "Theology" has begun to pay more attention to the philosophical issues raised by the paraphrase. 3 Here I hope to further this trend by suggesting that, although the author [End Page 211] of this paraphrase was not Aristotle, his own thought was suffused with Aristotelianism.First, we should note that the so-called "Theology of Aristotle" is itself only a part of an original, larger paraphrase of the Enneads. It is now usually thought that this paraphrase was authored by a member of al-Kindi's circle in ninth- century Baghdad. 4 The original Arabic Plotinus has been preserved in three separate texts. The first is the "Theology" (hereafter Th.A) which is itself subdivided into a prologue, a series of "headings" (ru'us), and ten chapters, each called a mimar, meaning "chapter" in Syriac. 5 This is by far the longest of the three texts. We also have the so-called Letter on Divine Science, formerly attributed to al-Farabibut shown by Kraus to belong to the Arabic Plotinus materials. 6 Finally, there are the Sayings of the Greek Sage, a set of fragments collected from various sources by Rosenthal and Lewis which also paraphrases Plotinus in Arabic. 7 As argued by F. W. Zimmermann, these three sources all derive originally from a lost, perhaps much more extensive, paraphrase of Plotinus. 8 They are united both by writing style and philosophical content. I will call them, collectively, AP; and since I cannot here provide conclusive arguments as to the identity of their author, I will call him the Adaptor.One of the major goals of al-Kindi and his circle seems to have been the synthesis of available Greek texts into a coherent philosophy. Thus al-Kindi himself produced doxographical works (for example, his Discourse on the Soul) and a survey of the Aristotelian corpus. 9 If the Adaptor worked in such a milieu, he may well have been influenced by other philosophical sources in making a paraphrase of Plotinus. Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

Neural substrates of fatigue in traumatic brain injury are not well understood despite the considerable burden of fatigue on return to productivity. Fatigue is associated with diminishing performance under conditions of high cognitive demand, sense of effort, or need for motivation, all of which are associated with cognitive control brain network integrity. We hypothesize that the pathophysiology of TBI results in damage to diffuse cognitive control networks, disrupting coordination of moment-to-moment monitoring, prediction, and regulation of behavior. We investigate the cingulo-opercular (...) and frontoparietal networks, which are engaged to sustain attention for task and maintain performance. A total of 61 individuals with mild TBI and 42 orthopedic control subjects participated in functional MRI during performance of a constant effort task requiring altering the amount of effort utilized to manually squeeze a pneumostatic bulb across six 30-s trials. Network-based statistics assessed within-network organization and fluctuation with task manipulations by group. Results demonstrate small group differences in network organization, but considerable group differences in the evolution of task-related modulation of connectivity. The mild TBI group demonstrated elevated CO connectivity throughout the task with little variation in effort level or time on task, while CO connectivity diminished over time in controls. Several interregional CO connections were predictive of fatigue in the TBI group. In contrast, FP connectivity fluctuated with task manipulations and predicted fatigue in the controls, but connectivity fluctuations were delayed in the mild traumatic brain injury group and did not relate to fatigue. Thus, the mTBI group’s hyper-connectivity of the CO irrespective of task demands, along with hypo-connectivity and delayed peak connectivity of the FP, may allow for attainment of task goals, but also contributes to fatigue. Findings are discussed in relation to performance monitoring of prediction error that relies on internal cues from sensorimotor feedback during task performance. Delay or inability to detect and respond to prediction errors in TBI, particularly evident in bilateral insula-temporal CO connectivity, corresponds to day-to-day fatigue and fatigue during task performance. Our think tank tasked by the Dutch Health Council, consisting of Radboud University Nijmegen Honours Academy students with various backgrounds, investigated the implications of Deep Brain Stimulation for psychiatric patients. During this investigation, a number of methodological, ethical and societal difficulties were identified. We consider these difficulties to be a reflection of a still fragmented field of research that can be overcome with improved organization and communication. To this effect, we suggest that it would be useful to found a centralized (...) DBS organization. Such an organization makes it possible to 1) set up and maintain a repository, 2) facilitate DBS studies with a larger sample size, 3) improve communication amongst researchers, clinicians and ethical committees, and 4) improve communication between DBS experts and the public at large. Kierkegaard, a poet of ideals and practitioner of the indirect method, also had a direct and polemical side. He revealed this in several writings throughout his career, culminating in The Moment, his attack against the established ecclesiastical order. Kierkegaard was moved to criticize the church by his differences with Bishop Mynster, Primate of the Church of Denmark. Although Mynster saw in Kierkegaard a complement to himself and his outlook, Kierkegaard challenged Mynster to acknowledge the emptying and estheticizing of Christianity that (...) had occurred in modern Christendom. For three years Kierkegaard was silent, waiting. When Mynster died, he was memorialized as "an authentic truth-witness" in the "holy chain of truth-witnesses that stretches through the ages from the days of the apostles." This struck Kierkegaard as blasphemous and inspired him to write a series of articles in Fædrelandet, which he followed with ten numbers of the pamphlet The Moment. This volume includes the articles from Fædrelandet, all numbers of The Moment, and several other late pieces of Kierkegaard's writing. (shrink)

Hannah Arendt's approach to politics focuses on action and conduct, rather than institutions, constitutions, and states. In light of Arendtian conceptions of politics, essays in this book challenge conventional IR theories. The contributions on agency explore concepts and categories of political action that enable individuals to act politically and to re-make the world in new, unpredictable ways. The contributions on structure explore how Arendt provides new critical purchase upon often reified structures and categories.

This paper has three purposes: first, to explicate the ex istential basis of Arendt's theory of action. This will be done by first tracing the intellectual derivation of Arendt's existentialism and the modifications she made to fit it in to her public realm. Second, I will demonstrate the con nection between Arendt's existentialism and her formula tion of political freedom. Third, I will illustrate throughout that Arendt's political ideas, if they are to be properly understood, must be subsumed under her (...) existentialism .\nPart I treats Arendt's effort to find a home for the in dividual Human Being by attacking the modern tendency to make freedom an inner domain of the mind and refuge from the outside world. We will see how she uses the ideas of Heidegger and Jaspers to counter the Being of Kant and the philosophy of Mill and Augustine. Part II demonstrates her effort to make freedom a world reality and counter to the forces of totalitarianism: the modern enchantment with totality and an inexorable history. She attacks the Will, consciousness, Being, and the historical process itself.\nFinding no suitable conceptualization of freedom which makes it political by giving it a public space, Arendt turns finally to Jaspers' communicative existentialism and Duns Scotus' emphasis on contingency. Many manipulation arguments against compatibilism rely on the claim that manipulation is relevantly similar to determinism. But we argue that manipulation is nothing like determinism in one relevant respect. Determinism is a "universal" phenomenon: its scope includes every feature of the universe. But manipulation arguments feature cases where an agent is the only manipulated individual in her universe. Call manipulation whose scope includes at least one but not all agents "existential manipulation." Our responsibility practices are impacted in different ways by (...) universal and existential phenomena. And this is a relevant difference, especially on Strawsonian approaches to moral responsibility, which take facts about our responsibility practices to be deeply connected to the nature of responsibility itself. We argue that Strawsonian accounts of moral responsibility are immune to manipulation arguments, and no attempt to modify the scope of manipulation or determinism featured in these arguments will help incompatibilists secure their desired conclusion. Olfaction has recently been highlighted as a sense poorly connected with language. Odor is difficult to verbalize, and it has few qualities that afford mimicry by vision or sound. At the same time, emotion is thought to be the most salient dimension of an odor, and it could therefore be an olfactory dimension more easily communicated. We investigated whether sounds imitative of an innate disgust response can be associated with unpleasant odors. In two experiments, participants were asked to make a (...) forced choice between a pseudoword including a disgust sound and a neutral pseudoword, for pleasant and unpleasant odors. Overall, participants chose more disgust pseudowords than neutral pseudowords for unpleasant odors, but this was not the case for pleasant odors. This effect was not driven by a general association between unpleasant sounds and unpleasant odors, but specifically the sounds [x/χ] and [f], thought to reflect physical responses to disgusting odors, as seen in the Polish fu! or the English ugh!. In line with growing evidence that language can encode odor, we provide the first experimental evidence for an association between the sound of a word and odor valence. (shrink)

A work that "not only treats of irony but is irony," wrote a contemporary reviewer of The Concept of Irony, with Continual Reference to Socrates. Presented here with Kierkegaard's notes of the celebrated Berlin lectures on "positive philosophy" by F.W.J. Schelling, the book is a seedbed of Kierkegaard's subsequent work, both stylistically and thematically.
